MANDATORY IMMUNIZATIONS FOR 6-TH-12TH GRADE STUDENTS
New immunizations will be required by the first day of school for
incoming 6th-12th graders. Failure to provide the school with this information can result in your child
being unable to return to school at that time. The immunizations are:
-Varicella ( Chickenpox) -2 doses, unless the child has had the chickenpox illness
-Tdap (Tetanus, Diptheria, and Pertussis)-1 dose after the 10th birthday
-MCV4 (Meningitis)-1 dose
You will be receiving a letter in the mail explaining what your child is needing. Please have these
completed and turned back into the school by August 11th, 2016. You can have these completed
through your physician or the Gibson County Health Department. The Health Department is open all
summer and has appointments on Thursdays to get the immunizations completed. You must call them at
385-3831 to schedule an appointment.
Please take a copy of the immunizations your child has received anywhere in the past with you.
Also, please turn in the CHIRP Immunization letter that is required for immunizations to be put in
electronically for state reports for accreditation, if you have not already done so. You can print the letter
off that is noted on the website and turn it into your child’s school.
