There is a fee charged for each copy. They may be requested by mail if the fee is enclosed. A copy of the certified birth certificate is made and placed in the student's permanent school record.

2009-2010 Highlights:
Parents & Students:
Wow, the summer went by way too fast! I hope your family enjoyed some vacation time together and your students are ready for another great year at Fort Branch Community School! The faculty and staff have been working hard to prepare for the students, and we are ready for an exciting year!
We have a few staff changes this year. Mrs. Sarah Edwards will be serving as our first grade teacher again this year. She has replaced Mrs. Mitz who resigned to stay at home with her children. Ms. Megan Tenbarge has replaced Mr. Jennings in fifth grade. Our thoughts and prayers are with Mr. Jennings’ family on his passing in June. He will be missed in the hallways this year at Fort Branch Community School. Mr. Isaac Birch will also be joining us in our elementary special education department. Ms. Quanty left us this year to move to Kentucky with her new husband. Mr. Ted O’Brien will be replacing her in middle school social studies. Mr. Brent Winstead and Mrs. Amanda Watson have joined us in our music department. Mr. Winstead will be serving as Fifth Grade and Middle School Band Director and Mrs. Watson will be teaching sixth grade General Music.
Over the coming year, I encourage you to stay involved with your child’s education through making sure they are present daily, assisting in homework, tracking student achievement through STI Home Plus, and communicating with teachers through the agenda, phone calls, and email! I also encourage you to become involved with our PTO through volunteering for some of our programs and student activities! I hope you and your child have a great school year, and I look forward to seeing you soon!
Mr. Michael Galvin

1. Please review the demographic page and make any corrections. One area which the state has changed this year is the “Race” field. Please review this area and contact the school secretary is it needs to be changed. Please return the form to your child’s homeroom teacher with your initials even if there are no corrections ASAP!

2. Please read and review the student handbook because some policies have changed this year. Pages 23 and 24 should be completed and remain in the student handbooks. Page 25 should be completed, removed from the Student Agendas, and returned to your homeroom teacher. The completed “Network Usage Agreement” must remain in the student agenda!

3. We are celebrating the first day of school with Mr. Isaac Birch and Mr. Robert Asa presenting a martial arts performance for our kindergarten through fifth grade students. Middle school students will be participating in goal setting workshops and team building exercises.

4. Students going home differently require a note from parents submitted to the office. If your child is going home a different way due to an emergency, please try to make arrangements prior to 2:00 P.M. It is difficult to communicate with teachers and students to ensure the change is known.

5. Book fees are due the first day of school to Mrs. McKee the school treasurer. Parents wishing to establish a payment schedule for their book fees must contact the office by Friday, August 14, 2009.

6. Breakfast, lunch and ice cream money must be sent in a sealed envelope with student’s name, homeroom teacher, grade level, and itemized break down printed on the front of the envelope. Beginning in September, the cafeteria will be implementing an automated system for all students. Student lunch accounts will be kept electronically, and parents will be able to review information online.

7. All students in grades 6, 7, and 8 are required to have a uniform for physical education. Students may use their same uniform from last year or T-shirts may be purchased through the PTO spiritwear. Black or gold shorts can be purchased through another store. The school dress code does apply to physical education uniforms.
